  1. Accomplish all mandatory fields on the Registration Form
  2. One Registration Form per member
  3. Please deposit payment to any  Bank of the Philippine Islands (BPI)

  4. Send scanned copy of bank validated deposit slip as proof of payment to secretariat@bcbpnac32.ph or fax to (02) 872-2966.  Attention: BCBP NAC32 Secretariat
  5. Indicate your name(s) on the face of the deposit slip for easy reference and monitoring
  6. Submission of this NAC32 Registration Form does not constitute registration unless accompanied by a validated deposit slip
